Abstract
Infeksi Saluran Kemih (ISK) merupakan salah satu jenis penyakit
infeksi yang sering ditemukan di pelayanan kesehatan dan biasanya
memerlukan pengobatan dengan antibiotik sebagai solusi utamanya.
Penggunaan antibiotik yang tidak tepat dapat mengakibatkan resistensi,
efek samping yang tidak diinginkan, dan peningkatan biaya terapi.
Evaluasi penggunaan antibiotik secara sistematis dan terencana perlu
dilakukan, salah satunya dengan metode Gyssens yang mengevaluasi
kesesuaian indikasi, jenis, dosis, interval waktu, dan lama penggunaan
antibiotik. Tujuan penelitian ini adalah untuk menilai rasionalitas
penggunaan antibiotik pada pasien infeksi saluran kemih di RSUD dr.
Gondo Suwarno pada periode Januari – Juni 2025.
Penelitian ini merupakan penelitian observasional dengan
pendekatan deskriptif evaluatif dengan menggunakan data sekunder dari
rekam medis pasien rawat inap. Analisis dilakukan berdasarkan kategori
Gyssens yang membagi penggunaan antibiotik menjadi kategori rasional
dan irasional berdasarkan kriteria tertentu. Diharapkan hasil penelitian
ini dapat memberikan wawasan mengenai pola penggunaan antibiotik
yang ada dan menjadi acuan untuk meningkatkan pengambilan
keputusan terkait terapi antibiotik yang lebih rasional di layanan
kesehatan.
Hasil penelitian menunjukkan bahwa antibiotik tunggal yang
paling banyak digunakan pada pasien ISK di RSUD dr. Gondo Suwarno
adalah ceftriaxone dan ciprofloxacin, sedangkan antibiotik kombinasi
terbanyak adalah ampisillin–sulbaktam. Dari 61 pasien, 63,93%
penggunaan antibiotik tergolong rasional, sementara 36,06% tidak
rasional berdasarkan metode Gyssens. Ketidakrasionalan tertinggi
terdapat pada kategori III B, diikuti kategori II A dan II B. Meskipun
sebagian besar terapi telah sesuai pedoman, evaluasi dan pengawasan
tetap diperlukan untuk mencegah kesalahan peresepan dan resistensi
antimikroba.
Urinary Ttract Infection (UTI) is one type of infectious disease that
is often found in health services and usually requires treatment with
antibiotics as the main solution. Inappropriate use of antibiotics can
result in resistance, unwanted side effects, and increased therapy costs.
Therefore, it is necessary to evaluate the use of antibiotics in a systematic
and planned manner, one of which uses the Gyssens method, which
evaluates the suitability of indications, types, doses, time intervals, and
duration of antibiotic use. The purpose of this study was to assess the
rationality of antibiotic use in patients with urinary tract infections at Dr.
Gondo Suwarno Hospital throughout January – june 2025.
This study was an observational study with an evaluative
descriptive approach using secondary data from the medical records of
hospitalized patients. The analysis was conducted based on the Gyssens
category, which divides antibiotic use into rational and irrational
categories based on certain criteria. It is hoped that the findings of this
study can provide insight into existing antibiotic use patterns and become
a reference for improving decision making related to more rational
antibiotic therapy in health services.
The results showed that the most commonly used single
antibiotics in UTI patients at dr. Gondo Suwarno Regional General
Hospital were ceftriaxone and ciprofloxacin, while the most commonly
used combination antibiotic was ampicillin–sulbactam. Of the 61
patients, 63.93% of antibiotic use was classified as rational, while
36.06% was irrational based on the Gyssens method. The highest
irrationality was found in category III B, followed by categories II A and
II B. Although most therapies were in accordance with guidelines,
evaluation and monitoring are still needed to prevent prescribing errors
and antimicrobial resistance.
