Abstract
Manajemen pengelolaan obat merupakan aspek penting dalam
pelayanan rumah sakit dan mencakup berbagai tahapan seperti pemilihan,
perencanaan, pengadaan, penerimaan, penyimpanan, pendistribusian,
penarikan dan pemusnahan, yang juga mencakup pencatatan dan
pelaporan. Obat High Alert merupakan obat yang dalam penggunaanya
perlu di waspadai karena dapat menyebabkan kejadian fatal apabila
dalam pemberian terjadi kesalahan. Tujuan penelitian adalah untuk
mengetahui sistem pengelolaan obat High Alert di Instalasi Farmasi
Rumah Sakit UNS yang meliputi: Perencanaan, pengadaan, penerimaan,
penyimpanan, pendistribusian, penarikan dan pemusnahan, serta
pencatatan dan pelaporan.
Metode yang digunakan dalam penelitian ini adalah deskriptif
kualitatif dengan pengumpulan data dilakukan melalui wawancara dan
observasi. Sampel yang digunakan berupa seluruh obat High Alert serta
7 Informan yang terdiri dari 3 Apoteker dan 4 TTK. Penelitian ini
dilakukan di bulan April – Mei tahun 2025.
Hasil penelitian menunjukkan bahwa perencanaan obat High
Alert di Instalasi Farmasi Rumah Sakit UNS menggunakan metode
konsumsi. Pengadaan dilakukan melalui pemesanan ke distributor sesuai
kebutuhan. Penerimaan obat mencakup pengecekan jumlah, nama, dosis,
bentuk sediaan, nomor batch, tanggal kedaluwarsa, dan kondisi fisik oleh
TTK. Penyimpanan obat High Alert dilakukan dengan rak terpisah dan
label khusus, meskipun metode Tall Man Lettering pada obat LASA
belum diterapkan. Distribusi dilakukan secara desentralisasi. Penarikan
dan pemusnahan obat kedaluwarsa dilakukan melalui pihak ketiga
karena ketiadaan fasilitas insinerator. Seluruh proses didukung dengan
pencatatan melalui kartu stok, dan pelaporan rutin obat-obatan khusus
seperti narkotika.
Medication management is an important aspect of hospital
services and includes various stages such as selection, planning,
procurement, receipt, storage, distribution, withdrawal, and disposal,
which also encompasses recording and reporting. High Alert drugs are
medications that need to be used with caution because they can cause
fatal incidents if there is an error in their administration. The purpose of
the research is to understand the management system of High Alert drugs
at the Pharmacy Installation of UNS Hospital, which includes: planning,
procurement, receipt, storage, distribution, withdrawal and destruction,
as well as recording and reporting.
The method used in this research is qualitative descriptive with
data collection conducted through interviews and observations. The
sample used consisted of all High Alert medications and 7 informants,
including 3 pharmacists and 4 pharmacy technicians. This research was
conducted in April – May 2025.
The research results show that the planning of High Alert
medications at the Pharmacy Installation of UNS Hospital uses the
consumption method. Procurement is carried out through ordering from
distributors as needed. The receipt of medication includes checking the
quantity, name, dosage, form, batch number, expiration date, and
physical condition by the TTK. The storage of High Alert medications is
done on separate shelves with special labels, although the Tall Man
Lettering method for LASA medications has not yet been implemented.
Distribution is carried out in a decentralized manner. The recall and
destruction of expired drugs are carried out through a third party due to
the absence of incinerator facilities. The entire process is supported by
recording through stock cards and regular reporting of special
medications such as narcotics.
