Abstract
Pemeriksaan darah lengkap merupakan pemeriksaan yang sering dilakukan. Alat Hematology Analyzer harus diperiksa secara rutin untuk memastikan akurasi dan mutu pemeriksaan. Sigma metrik belum digunakan untuk evaluasi kinerja pemeriksaan di laboratorium RSUD Ir. Soekarno Sukoharjo, oleh sebab itu perlu dilakukan analisis kinerja analitik alat Hematology Analyzer menggunakan Sigma metrik berdasarkan variasi TEa.
Metode penelitian yang digunakan adalah deskriptif kuantitatif dengan
menggunakan data PMI dan PME alat Hematology Analyzer pada tahun 2023
siklus 2. Nilai sigma setiap parameter dihitung dengan rumus Sigma = TEa – Bias
/ CV. Data TEa diambil dari empat pedoman berbeda. Koefisien variasi dihitung
dari data PMI. Bias dihitung dari selisih nilai target dengan hasil laboratorium dari data PME.
Hasil penelitian menunjukkan bahwa kinerja alat Hematology Analyzer
Sysmex XN-1000 di laboratorium RSUD Ir. Soekarno Sukoharjo didapatkan hasil
Poor pada parameter RBC, WBC, HCT, MCH, dan MCHC menurut semua pedoman. Hasil Good menurut CLIA 2024 dan Good & Excellent menurut Standards of Spanish pada parameter Hb. Parameter MCV kelompok alat mendapatkan hasil World Class pada Standards of Spanish. Disarankan penerapan Westgard Sigma Rules 8x untuk 2 level kontrol dan 6x untuk 3 level kontrol. Jika aturan 9x diganti dengan aturan 6x, maka pembagian pekerjaan menjadi 3 run dengan 3 kontrol per run.
Complete blood count test are frequently conducted. Hematology Analyzer
must be routinely checked to ensure accuracy and quality of the tests. Sigma
metric have not been used to evaluate examination performance at RSUD Ir.
Soekarno Sukoharjo. Therefore, it’s necessary to conduct an analysys of analytical
performance Hematology Analyzer using Sigma metric based on TEa variations.
A descriptive quantitative method was employed, using IQC and EQC data
from Hematology Analyzer in the second cycle of 2023. Sigma value were
calculated with formula Sigma = TEa – Bias / CV. TEa data came from four
guidelines. Coefficient variation is calculated from IQC data. Bias is calculated from difference between target value and laboratory result from EQA data. Results showed that Sysmex XN-1000 Hematology Analyzer at laboratory
RSUD Ir. Soekarno Sukoharjo performed poorly on RBC, WBC, HCT, MCH and
MCHC parameters in all guidelines. Good results according to CLIA 2024 and
Good & Excellent according to Standards of Spanish on Hb parameters. MCV
parameters achieved World Class in Standards of Spanish. Recommended
Westgard Sigma Rules include 8x rules for 2 control levels and 6x rules for 3
control levels. Using 6x rule instead of 9x rule allows for 3 runs with 3 control
each.
