Abstract
Hipertensi merupakan silent killer dengan gejala bervariasi pada
setiap individu. Banyaknya terapi obat yang digunakan akan
menimbulkan potensi adanya Drug Related Problems pada proses
terapinya. Penelitian ini bertujuan untuk mengetahui profil penggunaan
obat pada pasien geriatri dengan hipertensi di RSUD Sukoharjo,
mengetahui kejadian Drug Related Problems (DRPs) berdasarkan
kategori masalah domain efektivitas pengobatan dan kategori penyebab
domain pemilihan obat dan pemilihan dosis PCNE V9.1 yang terjadi
pada pasien geriatri dengan hipertensi di RSUD Sukoharjo.
Penelitian ini merupakan penelitian deskriptif non
eksperimental secara retrospektif dengan menganalisis potensi kejadian
Drug Related Problems (DRPs) berdasarkan PCNE V9.1. Kriteria
inklusi meliputi data rekam medik pasien yang terdiagnosa hipertensi,
pasien geriatri (60 – 74 tahun), dengan atau tanpa penyakit penyerta
atau komplikasi. Kriteria ekslusi meliputi data rekam medis rusak atau
tidak terbaca. Berdasarkan kriteria inklusi dan ekslusi didapat 39 pasien
geriatri dengan hipertensi yang memenuhi kriteria. Pedoman analisa
tatalaksana terapi mengunakan JNC 7, data yang diperoleh kemudian
dianalisa secara deskriptif dan disajikan dalam bentuk tabel
(persentase).
Hasil penelitian menunjukan terapi yang paling sering
digunakan adalah kombinasi 2 obat antara amlodipine dan captopril
(12,8%). Angka kejadian DRPs di RSUD Sukoharjo dengan kejadian
terapi obat tidak optimal (98,36%), gejala yang tidak diobati (1,64%),
obat tidak sesuai pedoman (17,98%), kombinasi obat (4,49%),
duplikasi obat (1,12%), pengobatan obat tidak lengkap (1,12%), dosis
rendah (7,87%), dosis tinggi (5,62%), regimen tidak cukup sering
(10,11%) dan regimen terlalu sering (51,69%).
Hypertension is a silent killer with symptoms varying in each
individual. The large number of drug therapies used will give rise to the
potential for Drug Related Problems in the therapy process. This study
aims to determine the profile of drug use in geriatric patients with
hypertension at Sukoharjo District Hospital, determine the incidence of
Drug Related Problems (DRPs) based on the problem category in the
treatment effectiveness domain and the cause category in the drug
selection and dose selection domain of PCNE V9.1 that occurs in
geriatric patients with hypertension at Sukoharjo District Hospital.
This research is a non-experimental, retrospective descriptive
study by analyzing the potential occurrence of Drug Related Problems
(DRPs) based on PCNE V9.1. Inclusion criteria include medical record
data from patients diagnosed with hypertension, geriatric patients (60 –
74 years), with or without comorbidities or complications. Exclusion
criteria include damaged or illegible medical record data. Based on the
inclusion and exclusion criteria, 39 geriatric patients with hypertension
met the criteria. Guidelines for analyzing therapy management using
JNC 7, the data obtained is then analyzed descriptively and presented in
table form (percentages).
The results of the study showed that the most frequently used
therapy was a combination of 2 drugs, amlodipine and captopril
(12.8%). The incidence of DRPs in Sukoharjo District Hospital with
the incidence of suboptimal drug therapy (98.36%), untreated
symptoms (1.64%), drugs not according to guidelines (17.98%), drug
combinations (4.49%), drug duplication (1.12%), incomplete drug
treatment (1.12%), low dose (7.87%), high dose (5.62%), insufficiently
frequent regimen (10.11%) and too often (51.69%).
