Abstract
Gagal jantung adalah ketidakmampuan fungsi jantung untuk
memompa darah secara maksimal sehingga terjadi penumpukan cairan
dalah tubuh. Pemberian kombinasi obat untuk terapi gagal jantung dapat
mengakibatkan potensi interaksi obat yang dapat mempengaruhi kualitas
pengobatan. Penelitian ini bertujuan untuk mengetahui karakteristik
pengobatan gagal jantung, potensi interaksi obat yang terjadi, dan
mengetahui strategi pengatasan interaksi obat yang terjadi pada
pengobatan gagal jantung.
Penelitian ini merupakan analisis deskriptif dengan desain
observasional analitik dan pendekatan retrospektif. Metode pengambilan
sampel dilakukan dengan purposive sampling dari rekam medis pasien
terdiagnosis gagal jantung. Identifikasi interaksi obat dilakukan
menggunakan aplikasi drug.com dan literatur stockley’s drug
interaction. Focused Group Discussion (FGD) dengan apoteker dan
dokter spesialis jantung dilakukan untuk mengetahui strategi pengatasan
interaksi obat yang terjadi.
Hasil penelitian menunjukkan bahwa karakteristik pasien gagal
jantung yaitu 14 laki-laki dan 8 perempuan. Pasien terbanyak pada
rentang usia 65-74 tahun, dengan lama perawatan terbanyak yaitu 1-5
hari. Jumlah potensi interaksi obat yang terjadi sebanyak 215 interaksi.
Tingkat keparahan interaksi secara minor yaitu 50 kasus (23%),
moderate 134 kasus (62%), dan mayor 31 kasus (14%). Pengatasan
potensi interaksi obat dapat dilakukan dengan kolaborasi antara dokter
apoteker dan perawat. Seperti pengawasan tekanan darah, pemberian
jeda pada tiap obat yang terdapat potensi interaksi, dan penyesuaian
dosis.
Pharmacotherapy plays an important role in achieving the goals of
heart failure therapy. The administration of combination drugs for heart
failure therapy can result in potential drug interactions that may affect
the quality of treatment. This study aims to determine the characteristics
of heart failure treatment, the potential drug interactions that occur, and
to identify strategies for managing drug interactions in heart failure
treatment.
This study is a descriptive analysis with an analytical observational
design and a retrospective approach. Sampling was conducted using
purposive sampling from medical records of patients diagnosed with
heart failure. Drug interactions were identified using the drug.com
application and Stockley’s Drug Interactions literature. Focused Group
Discussion (FGD) with pharmacists and cardiologists was conducted to
identify strategies for managing drug interactions that occur.
The characteristics of the heart failure patients are 14 men and 8
women. The majority of patients are in the age range of 65-74 years, with
the longest treatment period being 1-5 days. The number of potential
drug interactions that occurred was 215 interactions. The severity of the
interaction was minor, namely 50 cases (23%), moderate 134 cases
(62%), and major 31 cases (14%). Overcoming potential drug
interactions can be done through collaboration between doctors,
pharmacists and nurses. Such as monitoring blood pressure, giving a
break for each drug that has the potential for interaction, and adjusting
the dose.
